The Potential of Natural Products as Gastroprotective Agents Against Paracetamol-Induced Gastric Damage: A Systematic Review

Abstract

Paracetamol is a drug with analgesic and antipyretic effects commonly used to relieve pain. However, excessive use or improper administration can cause organ damage, including the stomach. High doses of paracetamol may lead to irritation of the gastric mucosa. Natural substances are known to have significant potential as gastroprotective agents, as their bioactive compounds can strengthen the mucosal layer. This study aims to identify the potential of natural substances as gastroprotective agents against paracetamol-induced gastric damage. The research employed a systematic review approach, with literature searched through PubMed Central, Google Scholar, and ScienceDirect. The research questions were formulated using the PICO framework, while the article selection process followed the PRISMA 2020 guidelines (Preferred Reporting Items for Systematic Reviews and Meta-Analyses). Articles that successfully passed the title and abstract selection were then assessed for risk of bias and eligibility using the Joanna Briggs Institute Critical Appraisal Checklist for Quasi-Experimental Studies. Out of a total of 2,578 articles identified, five articles met the criteria for further review. The findings indicate that natural substances containing flavonoids, tannins, alkaloids, saponins, and polyphenols exhibit gastroprotective properties. These compounds act as antioxidants and anti-inflammatory agents, strengthening the gastric mucosal layer and enhancing its protective secretions. Therefore, natural substances have the potential to serve as an alternative in preventing gastric damage caused by high-dose or long-term paracetamol use.
