Bronchoscopic Lung Volume Reduction with Endobronchial Valves as Emphysema Treatment

Abstract

Chronic Obstructive Pulmonary Disease (COPD) defined as obstruction of respiratory tract airflow which not fully reversible. Based on Riskesdas, in 2018 there was an increase in prevalence from 3,7% to 4,5%. In COPD with severe emphysema, standard treatment has yet to produce significant clinical improvement. Compared to other current modalities, several scientific evidences report Bronchoscopic Lung Volume Reduction with Endobronchial Valves (EBV) may provide significant clinical improvement. EBV is a therapeutic procedure using a one-way valve placed in the target bronchus. The placement of the valve can resolve hyperinflation caused by emphysema. EBV may be used as a therapeutic option in severe emphysema. However, its application is still a challenge because it requires strict patient selection criteria, adequate preparation, supporting facilities, personnel, and post-operative evaluation. This review summarizes the mechanism of action, evidence based medicine of efficacy and safety, patient selection criteria, and post-operative evaluation of EBV as emphysema management.
