Holistic Management Of Pulmonary Tb With Antituberculous Drugs Allergic, Type 2 Diabetes Mellitus And 1st Grade Obesity In A Women Aged 54 Years Old Through A Family Medicine Approach

Abstract

In 2020 as many as 1,3 million peoples in the world died due to TB. In 2021 Lampung Province was ranked 9th in Indonesia. One of the challenges in TB treatment is the side effects of major drugs such as OAT allergies. Allergic drug eruption or adverse cutaneous drug eruption is a hypersensitivity reaction of the body to drugs with skin manifestations that can be accompanied by mucosal involvement. Patients with Diabetes Mellitus (DM) have a tendency to be twice as likely to be infected with pulmonary TB disease than individuals without diabetes because the body is in an immunocompromised condition. Obesity is a condition in which a person's body has fat levels that are too high and can cause various health problems, one of which is DM.. Patients must know about their disease and have the awareness and motivation to complete treatment and lifestyle modifications so that treatment goals can be achieved. The study is a case report. Primary data were obtained through history taking, physical examination and home visits. Secondary data were obtained from the patient's medical record at the Puskesmas. Assessment is carried out based on a holistic diagnosis from the beginning, process, and end of the study quantitatively and qualitatively. Patient Mrs. T has a complaint of swollen lips and tongue accompanied by a red rash on the skin all over the body since 2 days ago. Three days ago, the patient came to the Puskesmas for control of TB treatment which had finished the intensive phase and moved to the advanced phase. After the intervention based on Evidence-Based Medicine with patient-centered, family approach and community medicine,  there was a decrease in clinical symptoms and an increase in the knowledge of patients and her families.
