Holistic Management of Uncontrolled Type 2 Diabetes Mellitus And Hypertension In Unmarried Adult Women through the Family Medicine Approach

Abstract

One of the PTM with the highest number of sufferers is Diabetes Mellitus Type 2 and Hypertension. Diabetes mellitus (DM) type 2 is one of the non-communicable diseases that is still a health problem in the world with an increase in the number of sufferers every year. The prevalence of hypertension in Indonesia based on Riskesdas 2018 increased by 34.1% compared to 2013 which was 25.8%. Through identifying the risk factors, clinical problems, conduct patient management holistically by applying family doctors according to evidence based medicine through a patient center, family approach, and community oriented.This study is a case report. Primary data were obtained through history taking, physical examination, supporting examination and home visits to assess the physical environment. Assessment based on a holistic diagnosis from the beginning, process, and end of the study qualitatively and quantitatively. Ms. L aged 48 years complained of feeling weak, easily thirsty, frequent urination at night at night and headaches. There is a history of diabetes and hypertension. Patients rarely exercise and physical activity, dietary patterns and eating habits are not appropriate. In the knowledge assessment, the initial score before the intervention was 5 (low) and after the intervention it was 10 (high). In the aspect of eating patterns there is a change in the number of calories and the type of food that is appropriate. In the aspect of physical activity there is a change in physical activity at least 3 x / week for 30 minutes. The diagnosis and management of these patients has been done holistically, patient centered, family approach and in accordance with national guidelines and based on evidence based medicine. There is a change in the knowledge and behavior of patients and families.
